Surgery Consent Form

Dear Client,

Please review all forms carefully. If you have any questions, feel free to call or email us at (706) 507-3400 or info@myvetsmart.com. On your pet’s surgery day, you will be required to review and sign an Authorization/Estimate Form. We will also confirm the best phone number to reach you during your pet’s surgery.

The night before surgery, please do not provide any food or treats after 10:00 p.m. Water is fine and may be left out. If your pet is currently taking any medications, vitamins, or injections, please withhold the morning dose unless otherwise instructed by the doctor.

At the time of your pet’s scheduled surgery, the doctor will address any remaining questions or concerns and collect the completed Authorization Form.

If you have elected to have any recommended blood tests performed, these will be collected and completed prior to the start of surgery if they have not already been done. If any concerns arise, the doctor may contact you at the phone number listed on the Authorization Form.

After the surgery, the doctor will process your payment, review discharge instructions with you verbally, and provide a written copy. If you do not understand any instructions, please do not hesitate to ask for clarification.
